What Short-term Rehabilitation Means at Cascades

Short-term rehabilitation is focused on one goal: helping you recover, regain independence, and return home safely. It may be the right next step following a joint replacement, hospitalization, stroke, illness, or injury that requires additional therapy and clinical support before returning home.

At Cascades, rehabilitation is integrated into your overall plan of care. Physical, occupational, and speech therapists work closely with your nurses and physicians to coordinate your treatment, monitor your progress, and adjust your care plan as your needs evolve. With therapy available up to seven days a week, every aspect of your stay is designed to keep your recovery moving forward.

✓   Therapy starts within 24 hours of admission

✓   Private and semi-private suites

✓   Physical, occupational, and speech therapy under one roof

✓   On-site outpatient clinic waiting for you after discharge

Short-term Rehab vs. Long-term Skilled Nursing

Three kinds of rehab, and which one you’re looking for

Hospital discharge planners use these terms interchangeably with families, and they mean different things. Cascades provides subacute short-term rehab and long-term skilled nursing; acute rehab happens in a hospital setting before you reach us.

Acute inpatient rehab  (Rehab Hospital) Short-Term Subacute Rehab Long-Term Skilled Nursing
Who it’s for Patients who can handle 3+ hours of intensive therapy a day right after a major event  Recovering from surgery, a hospital stay, or an injury Ongoing daily care needs
Typical length One to a few weeks Days to a few weeks Indefinite
Focus Intensive daily therapy under physician supervision  Daily therapy toward a discharge goal 24/7 nursing support
What’s next Often a step down to subacute rehab  Discharge home, with outpatient follow-up on-site Continued residency, or step-down if condition improves

If you were told you need “rehab” after a hospital stay and aren’t sure which kind, call (561) 454-2000 and admissions can tell you in one conversation.

FOR CASCADES RESIDENTS

If you already live here, recovery starts through your own door

After a planned procedure or a fall, there’s no facility search and no new intake paperwork. Residents of Independent Living, Assisted Living, and Memory Care simply move within the building, with the same care coordinators who already know their chart. Since you’re already here, your Rehabilitation stay starts without leaving home.

Norman Good

Rehab Patient

They taught me how to walk again.
When I got here, I needed help just to get from my bed to a wheelchair. The therapy team worked with me every day, and now I walk without any assistance at all. I’m so grateful to everyone at Physical Therapy, and to all the friends I made here along the way.

Arnold Greenberg

Physical, Occupational & Speech Therapy patient

I came to Cascades from a place that kept me in bed, using a Hoyer lift just to move. Dina and her team got me up and going from day one – she told me, ‘He won’t lay in bed here,’ and she meant it. Now I’m walking with a walker, transferring on my own, and even my speech has improved. This Physical Therapy department is amazing. Because of them, I’m finally going home.

Skilled Nursing & Rehabilitation Center Patient

I can walk longer distances every day.
My goal was to build my strength back up, and the team here helped me do exactly that. I’m walking farther than I could when I started, and I feel stronger every week. Thank you to Shelly, Karen, Jessica, and Daniel for all your help.

Frequently asked questions about short-term rehab in Delray Beach

Short-term rehab has a discharge goal from day one, you're here to get strong enough to go home, usually over a matter of weeks. Long-term skilled nursing is for ongoing daily care with no set end date. See the comparison above: Cascades runs both under one roof, so a change in your condition doesn't mean changing addresses.
